Hospital Discharge System Redesign
We worked with a team of innovative clinicians at a leading healthcare institution in the United States to completely redesign its patient discharge workflow and supporting technologies.
The newly developed discharge process starts from the moment a patient is admitted to the hospital. Members of a multidisciplinary care team jointly, and in collaboration with each other, contribute to the discharge documentation throughout a patient's hospital stay. The system provides vital tools that assist clinicians in this process, such as:
- Reconciliation of pre-admit, inpatient and discharge medications
- Intelligent nursing instructions editor with thousands of templates linked to patient problems and diagnoses
- Coded terminology-based search services
- Seamless integration with Order Entry, Rx, ambulatory, Common Data Repository and a number of other systems
- Advanced decision support and notification services
To support this new system we designed and developed a flexible and extensible clinical documentation framework which we reused to develop an Emergency Department Documentation system for the same institution.
